How to refer a client for drug and alcohol testing
In short
Call (888) 305-5422, tell us what testing needs to answer, and have your client sign a release naming who receives results. We schedule testing by appointment on the Westside, document chain of custody when needed, confirm positive screens through partner laboratories, and send results quickly to the people your client authorizes.

Request a callbackWho this page is for
Attorneys, therapists, psychiatrists, treatment programs, case managers and interventionists who want a client tested by a team that answers the phone. You make one call, we set up testing by appointment, and results come back quickly to the people your client authorizes in writing.
More background: testing for attorney referrals and a testing partner for clinicians and programs.
How to refer a client
- Call (888) 305-5422. Monday to Friday, 9 AM to 5 PM. You or your client can call. We work by phone, so a call is the fastest way to start.
- Tell us what testing needs to answer. Which substances, how often, whether testing must be random, and whether you need chain of custody documentation.
- Your client signs a release. It names you and anyone else who should receive results. Nothing is shared with anyone who is not on it.
- We schedule the first appointment. Your client brings a photo ID. There are no walk-ins.
- Results come to you. Quickly, and only to the people named in the release.
What to send us
- Your client’s name and the best phone number to reach them
- Your name, role and how you want to receive results
- The tests you want: urine drug screen, breath alcohol, EtG, or saliva on request
- Frequency, and whether testing should be random or on set days
- Any written requirement your client must meet, such as an order or program rule, so we can tell you plainly whether we can match it
- Whether chain of custody documentation is needed
- Medications your client takes, so results can be read in context (see our medication disclosure checklist)
Please do not send clinical records we did not ask for. We only need what testing requires.
Consent and release of results
Testing is consent based. Your client decides who receives results and signs a written release naming each person or office. Results go only to the people on that release. If your client changes or withdraws the release, we follow the new instruction.
Collection, documentation and confirmation
Chain of custody documentation
Available when an attorney, court, program or employer asks for it, recording who handled the sample from collection onward.
What chain of custody meansObserved collection
Collections can be observed when required by the referral.
Laboratory confirmation
Positive screens can be confirmed by our partner laboratories, which matters when a result is disputed.
About lab confirmationTests offered
Instant 12 panel urine drug test (standard testing includes fentanyl), breathalyzer, EtG alcohol test, and saliva testing on request.

Where and when testing happens
All testing is by appointment. Our office is at 12326 Venice Blvd, Los Angeles, CA 90066, open for testing Monday to Friday, 9 AM to 5 PM. Outside those hours, testing can be scheduled at other Blackburn locations on the Westside. Mobile collection at a client’s home or work is available for an additional fee.
Cost depends on the tests, frequency, lab confirmation, documentation and location. See what affects cost.
What we do not do
- We do not give legal advice and cannot promise that any court, agency or program will accept a particular result. Confirm requirements first.
- We do not diagnose. Results are information for you and your client, and clinical judgment stays with the treating professional.
- We do not perform DOT regulated testing.
When a client needs more than testing, we can point them to more support across Blackburn: therapy and psychiatric care at Blackburn Clinical Services, sober living at Blackburn Recovery Services, one-on-one mentoring for accountability and structure, and treatment scholarships through The Blackburn Foundation. See court ordered testing and how monitoring works.
Common questions
Can I make the referral by email or text?
Please call (888) 305-5422, Monday to Friday, 9 AM to 5 PM. We do not take referrals by text, and a phone call is the fastest way to get testing set up.
What does my client need to bring to the first appointment?
A photo ID. It also helps to bring a list of current medications and any written testing requirement from a court, program or employer.
Can the release name more than one professional?
Yes. Your client can name an attorney, a therapist, a program and others on the written release. Each person named receives results, and no one else does.
Can my client be scheduled the same week I refer them?
Call us and we will look at the schedule with you. All testing is by appointment, with office testing Monday to Friday and other Blackburn locations on the Westside outside office hours.
Do you collect samples outside Los Angeles?
Testing happens at our Venice Blvd office, other Blackburn locations on the Westside, or by mobile collection at home or work for an additional fee. Call to check whether mobile collection can reach your client's location.
