12 panel urine drug test
In short
A 12 panel urine drug test screens one sample for common drug classes, including opioids, stimulants, benzodiazepines and cannabis, with results in minutes. At Blackburn Monitoring in Los Angeles, positives that will be acted on are confirmed by a partner laboratory, and results go only to people you authorize.
A 12 panel urine drug test is the workhorse of most monitoring programs. It screens one sample for twelve drugs or drug classes at once, gives a result within minutes, and costs far less than sending every sample to a laboratory. At Blackburn it is the test most families, clinicians and attorneys ask for first.

What the 12 panel cup screens for
Our instant cup covers the substances people most often need to rule in or out, including:
- Opioids, including common prescription opioids
- Stimulants, such as cocaine, amphetamines and methamphetamine
- Benzodiazepines, a class of sedatives and anti anxiety medications
- Cannabis (THC)
- Other commonly misused drug classes
Cups from different makers combine panels differently, and we update our supplies over time. Our standard testing includes fentanyl. If another substance matters to you in particular, such as a specific prescription, ask us before your first test. We will tell you exactly what the current cup covers and whether a different test or a laboratory panel is a better fit.
How far back a urine test looks
Urine testing usually reflects use over the past few days. SAMHSA’s clinical guide for primary care, TAP 32, lays out typical urine detection times by drug, and the general pattern is consistent:
- For most drugs, occasional use is typically detectable for a matter of days, not weeks.
- Heavy, regular cannabis use can stay detectable much longer than a single use, because THC is stored in body fat and released slowly.
- Long acting medications, including some benzodiazepines, can also stay detectable longer than short acting ones.
These are ranges, not promises. The amount used, how often, body size, hydration and the cutoff level on the cup all change the answer. We do not estimate whether a specific person will pass or fail a specific test.
What a cutoff is
A urine screen does not report “any trace.” It reports positive only when the drug or its breakdown product is above a set concentration, called the cutoff. Federal workplace rules show how this works. Under the SAMHSA Mandatory Guidelines for urine, for example, the initial test cutoff for marijuana is 50 ng/mL with a 15 ng/mL confirmatory cutoff, and for cocaine it is 150 ng/mL with a 100 ng/mL confirmatory cutoff (SAMHSA, Federal Register 2023).
Our program is not a federal workplace program, and instant cups may use different cutoffs. The point is simple: a negative result means “below the cutoff,” and a positive screen means “above the cutoff on a screening test,” which is why positives are confirmed.
What happens at your appointment
- Book a time. Testing is by appointment, Monday to Friday, 9 AM to 5 PM, at 12326 Venice Blvd. If you are on a random program, you will get an electronic notice telling you when to report.
- Check in with a person. Bring a photo ID and a list of any medications you take.
- Give the sample. The collection follows the procedure your plan calls for, including chain of custody steps if your court, attorney or program requires them.
- Read the result. The screen develops in minutes. A coordinator reviews it with you.
- Share it with the right people. Results go only to the people you have authorized in writing, such as a parent, therapist or attorney.
If the screen is positive
A positive instant screen is a starting point for a conversation, not a verdict. The National Institute on Drug Abuse notes that screening tests should be followed by a more specific confirmation test, and that results should not be the only basis for care decisions (NIDA). When a result will be acted on, we send the sample to one of our partner laboratories for confirmation.
We also ask about prescriptions and recent medications. Some legitimate medications can affect a screen, and the right context changes what a result means.
Better together: pairing with an alcohol test
A 12 panel cup does not tell you about drinking. If alcohol matters to your plan, we usually pair the cup with an EtG alcohol test, which looks back over recent days, or a breath test for alcohol right now.
One test or an ongoing program
You can book a single test, but the 12 panel cup is most useful in a random testing program, where nobody knows the day in advance. Frequency is set case by case. Cost stays low because instant screens are inexpensive; what changes your total is how often you test, whether you add an alcohol test and whether lab confirmation or written reports are needed.

Common questions
Does the 12 panel test include fentanyl?
Yes. Our standard testing includes fentanyl. Panels vary by cup, so if another specific substance matters to you, ask us before your first test and we will tell you what the cup covers or recommend a laboratory test.
Will a 12 panel test show drinking?
No. A 12 panel urine cup screens for drugs, not alcohol. If alcohol matters, add an EtG test for recent days or a breath test for right now.
Will my prescription medication show up?
Some prescriptions can show on a screen. Bring a list of your medications so the result can be read correctly, and tell us about any new prescription before you test.
Can drinking a lot of water change the result?
Very diluted urine can make a screen unreliable, and programs often treat a dilute sample as a problem to repeat. Drink normally before your test.
Why does a negative result not mean zero?
Urine screens report positive only above a set concentration called the cutoff. A negative means the level was below that cutoff, not that no trace of any substance existed.
