EtG alcohol test

In short

An EtG test checks urine for ethyl glucuronide, a marker the body makes when it processes alcohol, so it can show drinking over recent days rather than only the last few hours. Blackburn Monitoring offers EtG testing by appointment in Los Angeles, with partner laboratory confirmation when results will be acted on.

Alcohol leaves the breath within hours. EtG lets a test look back further. Ethyl glucuronide (EtG) is a substance the body makes when it processes alcohol, and it stays in urine well after the alcohol itself is gone. That makes EtG the test of choice when the question is not “is this person drunk right now” but “has this person been drinking.”

How long EtG can detect drinking

You will often hear EtG called a “72 hour” alcohol test. That is a useful rule of thumb, but the real window depends on how much someone drank and on the cutoff the test uses.

  • Mayo Clinic Laboratories states that EtG can be detected in urine for up to 5 days at a 500 ng/mL cutoff.
  • A 2015 study of addiction treatment patients by Lowe and colleagues found that lower cutoffs catch more drinking over time. The best performing cutoffs were about 420 ng/mL for drinking in the past 24 hours, 150 ng/mL at 48 hours and 100 ng/mL at 72 hours.
  • The same study found that the common 500 ng/mL commercial cutoff missed more drinking than lower cutoffs, and suggested about 200 ng/mL when looking back beyond one day.

In plain terms: heavy drinking can show for several days, and light drinking may fade sooner. No one can promise how long a specific drink will show for a specific person.

Incidental exposure: hand sanitizer, mouthwash and more

Because EtG is so sensitive, small amounts of alcohol from everyday products can sometimes produce a detectable result. Mayo Clinic Laboratories notes that incidental exposure from hand sanitizer and mouthwash can produce detectable EtG and its companion marker, EtS (Mayo Clinic Laboratories).

If you are on an EtG program, we will give you a short list of products to watch, starting with alcohol based hand sanitizer and alcohol containing mouthwash, plus any other product with alcohol on its label. Use alcohol free versions where you can, and tell us about anything you used before a test. Low level results are read with this in mind, and a coordinator will talk with you rather than jump to conclusions.

EtG and breath tests work as a pair

A breathalyzer answers whether alcohol is in the body now. An EtG test answers whether someone has been drinking over the last few days. Many plans use both: breath for the moment, EtG to cover the gaps between appointments. If drugs matter too, we add a 12 panel urine cup to the same visit.

SAMHSA’s advisory on alcohol biomarkers makes a related point: biomarkers like EtG work best alongside conversation and screening, not in place of them (SAMHSA advisory, summarized by RTI). That is how we use them.

Who uses EtG testing

  • People in recovery from alcohol use

Regular EtG results build a track record you can share with family, a therapist or a sponsor.

  • Families

A clear, objective answer can replace arguments about whether someone has been drinking.

  • Court and attorney referrals

Court ordered testing often includes alcohol. We can provide chain of custody documentation. Ask your attorney whether a specific vendor or format is required.

  • Clinicians

With consent, results go to the treating clinician so care can be adjusted early.

Confirming an EtG result

If an EtG screen is positive and the result will be acted on, we can send the sample to a partner laboratory for confirmation. Laboratory methods measure EtG and EtS precisely, which helps separate drinking from incidental exposure. Mayo Clinic Laboratories, for example, lists confirmation cutoffs of 250 ng/mL for EtG and 100 ng/mL for EtS by LC-MS/MS (Mayo Clinic Laboratories). Our partner laboratories’ methods and cutoffs may differ, and we will tell you what applies to your sample.

Booking an EtG test

EtG testing is by appointment at 12326 Venice Blvd, Los Angeles, Monday to Friday from 9 AM to 5 PM, with other Blackburn locations on the Westside available outside office hours and mobile testing at home or work for an additional fee. For most people, EtG is most useful on a random schedule a few times a week or more, set case by case with your coordinator.

When drinking is more than a test result

If EtG results keep coming back positive, that is information, not a failure. The National Institute on Alcohol Abuse and Alcoholism’s Rethinking Drinking site is a good place to look honestly at drinking patterns. Your coordinator can also connect you with therapy and psychiatric care or more structured support across Blackburn.

Need more than testing?

Monitoring is the first step, not the last. If a result or a hard week tells you more help is needed, Blackburn can connect you to it.

Your monitoring coordinator can make the introduction.

Common questions

Is the EtG test really 72 hours?

Seventy two hours is a common rule of thumb. The real window depends on how much was consumed and the cutoff used; Mayo Clinic Laboratories states up to 5 days at a 500 ng/mL cutoff, and light drinking may fade sooner.

What should I avoid before an EtG test?

Incidental exposure from products like hand sanitizer and mouthwash can produce detectable EtG, according to Mayo Clinic Laboratories. Use alcohol free products where you can and tell us what you used before testing.

Should I get an EtG test or a breathalyzer?

A breathalyzer shows alcohol in the body right now. EtG shows drinking over the past few days. Many plans use both.

Does EtG measure how drunk someone was?

No. EtG shows that alcohol was consumed within a window of time. It does not measure impairment or a past blood alcohol level.

How often should EtG testing happen?

Because the window is a few days, EtG is most useful on a random schedule a few times a week or more. Your coordinator sets frequency case by case.
